Who is Eligible to make an Appointment?

  • Patients must live in Waukesha County.

  • Patients must have no dental insurance and have income that falls between the 100% and 200% of the federal poverty guidelines.

  • Patients must be currently enrolled in BadgerCare Plus or Medicaid Title 19.

Services Provided:

 Pregnant Women and Children 18 and younger receive:

  • Comprehensive and emergency oral exams

  • X-rays

  • Cleanings including oral hygiene instruction

  • Fluoride treatments and sealants

  • Restorative services (fillings), extractions

  • Limited endodontics (root canals)

  • Prosthetics as appropriate

Adults receive:

  • Emergency care for pain and/or infection, which includes antibiotics and pain relief

  • Extractions

  • We do not do any cleanings, fillings, or denture work for adults

How Does a Patient Make an Appointment?

  • Patients must call the clinic to make an appointment.

  • The phone number to call is 262-522-7645.

How Does a Patient Register When at the Clinic?

  • Patients must bring proof of living in Waukesha County (such as a driver's license, electric bill or -water bill with current address) and photo identification.

  • Patients must provide proof of income (three recent paycheck stubs and W2 or previous year's tax return) or have a Forward card

  • Patients must bring medical history information with them, including any medications they are taking.

Will there be emergency appointments available every day?

  • The clinic will take patients with an emergency dental need as needed.  We ask that a patient or referral source call 262-522-7645, ext. 222 to verify a dentist will be available to treat emergency patients and to check in frequently as to the availability of appointments


 

WCCDC Attendance Policies

  • Patients who have one no show cannot reschedule for 6 months; a second no show will result in termination of services at WCCDC.           

  • Patients who do not call to cancel an appointment the day before the appointment or do not leave a message on the cancellation line will be designated a no show.           

  • Patients are responsible to call WCCDC with phone number changes.  WCCDC will cancel appointments for patients with disconnected numbers.           

  • Patients who no show due to an emergency may bring in appropriate documentation to be removed from the no-show list.

  • Patients who arrive more than 10 minutes late for an appointment will be rescheduled.  A second tardy visit will be considered a no-show.           

  • Uninsured patients must bring proof of family income and residency to each appointment.

  • Patients with the Forward Card must bring the card to each appointment.
